My first name is 安叶芝 or An Ye Zhi, it is the name that is in my passport.
An is my given name by my parents, it is inspired from the character 安 but also by the nicknames for my aunt, grandma and great grandma, named: Anna, Annet and Annie. 
Ye Zhi or 叶芝 is the name given by the orphange I am from. Added to this in my original name was also the character Gong. My parents removed this part. 

From a young age I could write the character 安, but not 叶芝. I never went by my full first name. My roepnaam, the way people call me in the Netherlands, is An. In China they also call me An, but it is more considered as an English name, rather then connected to my full Chinese name. I cannot pronounce my Chinese name correctly. So, I at least learned how to write it
